To register please complete your medical card and hand in at the Health Centre. If you do not have this card, a form can be provided by our reception staff.
You will be registered with the practice, not an individual GP. You may however, select a GP you would prefer to be seen by, but circumstances may not always make this possible.
If you have the latest version of Adobe Reader please click here to download a registration form. You will then be able to complete the form on you computer. When complete, print the form and bring it into the practice.

A form of identification is now needed to register with the practice and that can be either:
Contract of Employment/Letter from Employer
Letter from the Home Office / UK Border Agency (UKBA)
Proof of Residence
Student ID Card or Letter from Education Facility
European Health Insurance Card (EHIC)
Documentation from Job Centre Plus or the Department of Work and Pensions (DWP)
Photo Drivers Licence
If you would like to register for our range of Online Services currently in development please download our Online Services Registration form and return it to the medical Practice in person, along with a valid form on identification. Once you are registered the practice will give you the infomation that will enable you to create a username and password. For further information on our online services click here
New Registration Medicals
We offer all new patients an appointment to welcome them to the Health Centre, to provide a health check and discuss any previous or present medical problems. We strongly encourage new patients to use this service to improve your care as it takes more than 10 weeks for your notes to arrive here from your previous practice. An appointment can be made at reception at time of registration.
